Son y\u0131llarda hedefe y\u00f6nelik tedaviler ve imm\u00fcnoterapi alan\u0131nda ciddi geli\u015fmeler kaydedildi\u011fini s\u00f6yleyen Prof. Dr. \u0130lhan \u00d6ztop, bu y\u00f6ntemlerle ya\u015fam s\u00fcrelerinde \u00f6nemli kazan\u0131mlar sa\u011fland\u0131\u011f\u0131n\u0131 vurgulad\u0131.<\/h2>\n<p>European Society for Medical Oncology, T\u00fcrk T\u0131bbi Onkoloji Derne\u011fi, Anadolu T\u0131bbi Onkoloji Derne\u011fi ve T\u00fcrk Akci\u011fer Kanseri Derne\u011fi \u00fcyesi, Dokuz Eyl\u00fcl \u00dcniversitesi T\u0131p Fak\u00fcltesi T\u0131bbi Onkoloji Bilim Dal\u0131 Ba\u015fkan\u0131 <strong>Prof. Dr. \u0130lhan \u00d6ztop<\/strong>, d\u00fcnya genelinde en \u00f6nemli kanser istatistikleri kayna\u011f\u0131 olan GLOBOCAN verilerine ve D\u00fcnya Sa\u011fl\u0131k \u00d6rg\u00fct\u00fc kaynaklar\u0131na g\u00f6re 2012 y\u0131l\u0131nda kaydedilen toplam kansere ba\u011fl\u0131 \u00f6l\u00fcm say\u0131s\u0131n\u0131n 8,2 milyon oldu\u011funu ve bunlar\u0131n 1,5 milyonunun akci\u011fer kanseri kaynakl\u0131 oldu\u011funu vurgulad\u0131. 2025 y\u0131l\u0131nda toplam kanser vakas\u0131 say\u0131s\u0131n\u0131n 22 milyona ula\u015fmas\u0131n\u0131n beklendi\u011fini belirten Prof. Dr. \u0130lhan \u00d6ztop, akci\u011fer kanserinin yayg\u0131nl\u0131\u011f\u0131n\u0131 \u015f\u00f6yle a\u00e7\u0131klad\u0131: \u201cD\u00fcnya\u2019da en \u00e7ok tan\u0131 konulan kanserler s\u0131ras\u0131yla akci\u011fer, meme ve kolon kanseri iken;\u00a0 kansere ba\u011fl\u0131 \u00f6l\u00fcmlerin s\u0131ras\u0131yla en \u00e7ok akci\u011fer, karaci\u011fer ve mide kanserinde ger\u00e7ekle\u015fti\u011fi bildirilmektedir. Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131 verilerine g\u00f6re \u00fclkemizde en s\u0131k g\u00f6r\u00fclen kanser t\u00fcrleri erkeklerde s\u0131ras\u0131yla akci\u011fer, prostat ve mesane kanseri; kad\u0131nlarda ise meme, tiroid ve kolorektal kanserdir. En s\u0131k \u00f6ld\u00fcren kanser t\u00fcrleri ise erkeklerde s\u0131ras\u0131yla akci\u011fer, prostat ve kolorektal kanserler iken; kad\u0131nlarda akci\u011fer, meme ve kolorektal kanserlerdir. G\u00f6r\u00fcld\u00fc\u011f\u00fc \u00fczere akci\u011fer kanseri mortalitesi (\u00f6l\u00fcm oran\u0131) y\u00fcksek bir kanser olup her iki cinste de en \u00f6ld\u00fcr\u00fcc\u00fc kanser olarak ilk s\u0131rada yer almaktad\u0131r.\u201d<\/p>\n<p><strong>Umut veren tedaviler: ki\u015fiye \u00f6zel tedavi ve imm\u00fcnoterapi<\/strong><\/p>\n<p>Akci\u011fer kanserinin tedavisinde cerrahi ve radyoterapi alan\u0131nda kaydedilen ilerlemelerin yan\u0131 s\u0131ra medikal onkoloji alan\u0131nda da son y\u0131llarda \u00f6nemli geli\u015fmeler oldu\u011funu belirten Prof. Dr. \u0130lhan \u00d6ztop, \u015funlar\u0131 s\u00f6yledi: \u201cEskiden akci\u011fer kanserleri k\u00fc\u00e7\u00fck h\u00fccreli ve k\u00fc\u00e7\u00fck h\u00fccreli olmayan akci\u011fer kanseri ba\u015fl\u0131klar\u0131 alt\u0131nda de\u011ferlendirilip tedavi edilirken, g\u00fcn\u00fcm\u00fczde k\u00fc\u00e7\u00fck h\u00fccreli olmayan akci\u011fer kanserinin histolojik alt tiplerinin \u00f6nemi ortaya \u00e7\u0131km\u0131\u015ft\u0131r. Buna ilaveten, t\u00fcm\u00f6r\u00fcn geli\u015fmesinde rol oynayan spesifik mutasyonlar\u0131n saptanmas\u0131n\u0131 sa\u011flayan molek\u00fcler analizler \u00f6nem kazanm\u0131\u015f ve akci\u011fer kanserinin geli\u015fiminde kritik rol oynayan birtak\u0131m mutasyonlar\u0131n varl\u0131\u011f\u0131 ara\u015ft\u0131r\u0131larak, bu mutasyonlara y\u00f6nelik spesifik tedavilerin kullan\u0131lmas\u0131 imkan\u0131 do\u011fmu\u015ftur. Ayr\u0131ca yine yak\u0131n zamanda imm\u00fcnoterapi alan\u0131nda da geli\u015fmeler olmu\u015ftur. Bu tedaviler ba\u011f\u0131\u015f\u0131kl\u0131k sisteminin de\u011fi\u015fik noktalar\u0131nda etki ederek v\u00fccudun ba\u011f\u0131\u015f\u0131kl\u0131k sisteminin kansere kar\u015f\u0131 yan\u0131t\u0131n\u0131 art\u0131rmaktad\u0131r. Akci\u011fer kanseri tedavisinde \u00f6nemli katk\u0131lar sa\u011flayan bu y\u00f6ntemin yak\u0131n zamanda h\u0131zla yayg\u0131nla\u015fmas\u0131 beklenmektedir.\u201d<\/p>\n<p><strong>Ki\u015fiye \u00f6zel tedavi: mutasyona \u00f6zel tedavi<\/strong><\/p>\n<p>Son y\u0131llarda hedefe y\u00f6nelik tedaviler alan\u0131nda \u00e7ok \u00f6nemli geli\u015fmeler kaydedildi\u011fini belirten Prof. Dr. \u0130lhan \u00d6ztop, bu tedavi y\u00f6ntemlerini \u015f\u00f6yle a\u00e7\u0131klad\u0131: \u201cKi\u015fiye \u00f6zel, yani hedefe y\u00f6nelik tedavilerde, kanserin geli\u015fiminde ve yay\u0131lmas\u0131nda rol oynayan baz\u0131 yolaklardaki spesifik mutasyonlar, molek\u00fcler y\u00f6ntemlerle saptanmakta ve bunlara uygun ila\u00e7lar kullan\u0131lmaktad\u0131r. B\u00f6ylece kanserli h\u00fccrenin \u00e7o\u011falmas\u0131nda kritik rol oynayan k\u0131s\u0131mlar bloke edilmektedir. Meme ve mide kanserindeki HER-2 yola\u011f\u0131, kolon kanserindeki EGFR ve Anjiyogenez yolaklar\u0131 ve akci\u011fer kanserindeki EGFR ve ALK yolaklar\u0131 ile ilgili geli\u015fmeler bunlardan birka\u00e7\u0131 olup, t\u00fcm bu yolaklara y\u00f6nelik tedaviler hastalar\u0131n ya\u015fam s\u00fcrelerine anlaml\u0131 oranda katk\u0131 sa\u011flam\u0131\u015ft\u0131r.<\/p>\n<p><strong>En \u00f6nemli geli\u015fme k\u00fc\u00e7\u00fck h\u00fccreli olmayan akci\u011fer kanserlerinde sa\u011fland\u0131<\/strong><\/p>\n<p>Hedefe y\u00f6nelik tedavilerle ilgili en dramatik geli\u015fmelerin akci\u011fer kanserinin k\u00fc\u00e7\u00fck h\u00fccreli olmayan akci\u011fer kanseri alt tipinde oldu\u011funu s\u00f6yleyen Prof. Dr. \u0130lhan \u00d6ztop \u015f\u00f6yle devam etti: \u201cBu grup hastalar\u0131n \u00f6nemli bir k\u0131sm\u0131nda t\u00fcm\u00f6r\u00fcn \u00e7o\u011falmas\u0131na neden olan \u00e7ok say\u0131da spesifik mutasyonlar saptanmaktad\u0131r. Bunlardan bug\u00fcn i\u00e7in \u00fczerinde en fazla \u00e7al\u0131\u015f\u0131lan ve buna uygun tedavilerin geli\u015ftirilip kullan\u0131ma sunuldu\u011fu mutasyonlar EGFR, ALK ve ROS-1 mutasyonlar\u0131d\u0131r. Ayr\u0131ca t\u00fcm\u00f6r\u00fcn kanlanmas\u0131 ve beslenmesini engellemeye y\u00f6nelik olarak anjiyogenez inhibit\u00f6rleri geli\u015ftirilmi\u015ftir. \u00d6zellikle EGFR, ALK ve ROS-1 mutasyonlar\u0131 saptanmas\u0131 durumunda bunlara y\u00f6nelik uygulanan spesifik tedaviler hastalar\u0131n ya\u015fam s\u00fcrelerini ciddi boyutta art\u0131rm\u0131\u015ft\u0131r.\u201d<\/p>\n<p><strong>Tedavi s\u00fcresince i\u00e7ilen sigara iyile\u015fme \u015fans\u0131n\u0131 azalt\u0131yor<\/strong><\/p>\n<p>Prof. Dr. \u0130lhan \u00d6ztop, kansere neden olan fakt\u00f6rler aras\u0131nda t\u00fct\u00fcn kullan\u0131m\u0131 \u00f6nemli bir yer tuttu\u011funu belirtti ve t\u00fct\u00fcn kullan\u0131m\u0131 hakk\u0131nda \u015funlar\u0131 vurgulad\u0131: \u201cT\u00fct\u00fcn kalp-damar hastal\u0131klar\u0131 ve KOAH gibi ciddi hastal\u0131klara yol a\u00e7mas\u0131n\u0131n yan\u0131 s\u0131ra ba\u015fta akci\u011fer kanseri olmak \u00fczere bir\u00e7ok kansere neden olmaktad\u0131r. Akci\u011fer kanserinin %90\u2019\u0131nda t\u00fct\u00fcn rol oynamaktad\u0131r. Akci\u011fer kanserinin t\u00fcm d\u00fcnyada hem erkeklerde hem de kad\u0131nlarda en \u00f6ld\u00fcr\u00fcc\u00fc kanser olmas\u0131 t\u00fct\u00fcn ile m\u00fccadelenin \u00f6nemini bir kat daha artt\u0131rmaktad\u0131r. Bu anlamda \u00fclkemizde yasal d\u00fczenlemeyle belirli mekanlarda t\u00fct\u00fcn kullan\u0131m\u0131n\u0131n yasaklanmas\u0131 da toplum sa\u011fl\u0131\u011f\u0131 a\u00e7\u0131s\u0131ndan \u00e7ok \u00f6nemli bir ad\u0131m olmu\u015ftur. Sigara i\u00e7en ki\u015filerde kanser geli\u015fme riski artmakla kalm\u0131yor, beraberinde birden fazla alanda da kanser geli\u015febiliyor. Ayr\u0131ca kanser tedavisi s\u0131ras\u0131nda sigara i\u00e7ilmesine devam edilmesi halinde sigara tedavinin ba\u015far\u0131s\u0131n\u0131 azaltabiliyor. Ayr\u0131ca sigara i\u00e7meyenlerde g\u00f6r\u00fclen akci\u011fer kanseri vakalar\u0131nda, yukar\u0131da bahsedilen mutasyonlara daha fazla rastlan\u0131yor.
